Middlesex FA is committed to making football accessible for all.

As the most diverse county in the country, we want to ensure Football in Middlesex is reflective and representative of the community we serve.

As part of this commitment we have a rolling plan to collect data on the profile of our board, staff (paid and unpaid), coaches, officials and members/participants. Without this data it will not be possible to identify any current areas of under-representation or potential inequalities, and as such, it will make it much more difficult for us to tackle these issues.

The monitoring information will also greatly aid our efforts to ensure that all board, staff, coaches, officials and members/participants are treated fairly regardless of age, race, disability, sex, gender reassignment, sexual orientation, marriage and civil partnership, religion or belief and sexual orientation

This work is being carried out as part of our responsibilities under the Equality Act 2010 and as part of the accreditation process for the Preliminary Level of the Equality Standard for Sport.

The following survey should take no longer than 5 minutes to complete and all entries will remain anonymous and confidential, therefore information cannot be traceable. Only people analysing the data will have access to the information provided.

All questions are mandatory, however there is a 'prefer not to say' option should you wish not to share information about yourself.
